Providers
B&R works with patients and their Primary Care Providers to provide wound care in the patient's home. B&R does not provide Primary Care, so providers can refer to B&R without worrying about loosing patients. Providers fill out B&R Wound Referral Form found below and fax or Secure email to information found on form . Please include nay progress notes from the last 30-days regarding the patient's Chronic or other non-healing wound, and demographics for the patient including insurance.
Medicare defines a Chronic wound as, "Chronic wounds are wounds that have failed to proceed through an orderly and timely series of events to produce a durable, structural and cosmetic closure".
not all wounds qualify patients for service with B&R. Contact B&R for questions regarding type of wounds/codes that qualify.
ILF, B&C, SNF, ALF or Other RCFE
